Person Centred Planning

Person Centred Planning

Person-centred planning is a way of thinking and doing things that sees the people using health and social services as equal partners in planning, developing and monitoring care to make sure it meets their needs. This means putting people and their families at the centre of decisions and seeing them as experts, working alongside professionals within the disability sector to get the best outcome. The aim is to understand what each individual person wants and needs to live their own, personally well-defined life.

Person centred practice is also one of the keystones of Individualised support.
